Please complete the following form to refer a patient.

To send referrals (EPC/TCA, EDCP, D904, please send via Medical Objects).

If you are referring an NDIS participant, please note that Core Dietetics does not accept NDIA managed participants. If the participant is self or plan managed and has dietetics covered in their plan, you can proceed with completing this referral form.

Please note our preferred method of receiving referrals is via Medical Objects.
